HPC     became very sleepy

about 15 minutes ago

now only groaning when trying to wake him up

diarrhoea and tummy pain for 2 days

loss of appetite today, didn’t eat well

no vomiting, no fever

PMH    hypertension

stroke 5 years ago, no residuals

diabetes (tablet controlled)

gout

DH       glibenclamide

rosiglitazone

Aspirin

SH       lives with wife

retired car mechanic

smokes 3-4 cigarettes / day

drinks wine occasionally

 

FH       brother: heart attack
