PC       road traffic accident (RTA)

 

HPC     was hit by car when crossing street

can’t remember many details

no  loss of consciousness

severe pain in epigastrium and pelvis

painful shin and unable to move left foot

can’t stand up anymore

 

PMH    diabetes mellitus type 2

ischaemic heart disease (IHD)

- heart attack in 1988

- now still occasional chest tightness

osteoarthritis in both wrists

kidney stones (not causing problems currently)

 

DH       Aspirin

Diclofenac

Bisoprolol

Metformin

 

SH       lives in residency

retired social worker

smokes 2-3 / day

daily glass of sherry

FH       mother – tuberculosis

son – prostate cancer

daughter - melanoma
